OTHER USES AND DISCLOSURES For all other uses and disclosures of PHI, we will obtain your written authorization and will only use or disclose pursuant to that authorization. You may revoke such authorization in writing at any time. YOUR HEALTH INFORMATION RIGHTS - Request restrictions on certain uses and disclosures. - Request confidential communications at alternate locations. - Inspect and obtain copies of your PHI. - Request amendments to your PHI. - Receive an accounting of disclosures. - Request additional copies of this Notice. REVISIONS TO THIS NOTICE We may revise this Notice and make the new version applicable to all PHI maintained. The revised Notice will be posted in the pharmacy and available upon request. COMPLAINTS If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Secretary of Health and Human Services.
